Ces macromol\u00e9cules poss\u00e8dent de <strong>multiples r\u00f4les cl\u00e9 pour le bon d\u00e9veloppement du nourrisson [2]<\/strong>. Aussi, il est int\u00e9ressant d\u2019int\u00e9grer les HMO dans l\u2019alimentation infantile.<\/em><\/p>\n<p><em>Le nombre d&#8217;\u00e9tudes sur ces macromol\u00e9cules se multiplie, les principaux acteurs du march\u00e9 du lait infantile les int\u00e8grent \u00e0 leurs formulations infantiles et le nombre de march\u00e9s autorisant leur utilisation croit (avec la Chine comme dernier exemple en date ayant autoris\u00e9 l&#8217;utilisation de certains HMO dans l&#8217;alimentation infantile).<\/em><\/p>\n<p>&nbsp;<\/p>\n<h2><span style=\"text-decoration: underline;\">Les HMO : de quoi parle-t-on<\/span> ?<\/h2>\n<p>&nbsp;<\/p>\n<h3>Les HMO du lait maternel : des mol\u00e9cules abondantes et d\u2019une grande diversit\u00e9 [2]<\/h3>\n<p>De leur nom complet <strong>Human Milk Oligosaccharides<\/strong>, les HMO sont des mol\u00e9cules de la famille des glycanes, <strong>pr\u00e9sentes en abondance dans le lait maternel des diff\u00e9rents mammif\u00e8res<\/strong>. Toutes les femmes ne synth\u00e9tisent pas les m\u00eames types d\u2019oligosaccharides. En fonction du cycle de lactation, du patrimoine g\u00e9n\u00e9tique et de l\u2019alimentation maternelle, la teneur et la composition en HMO du lait maternel peut varier. [3]<\/h3>\n<p>Le microbiote se constitue lors des premiers mois de vie du nourrisson. Le d\u00e9veloppement d\u2019une flore intestinale riche, diversifi\u00e9e et \u00e9quilibr\u00e9e \u00e9tant d\u00e9terminant pour la sant\u00e9 physique et psychologique, il est n\u00e9cessaire d\u2019\u00eatre attentif \u00e0 l\u2019alimentation des nouveau-n\u00e9s.<\/p>\n<p>Il existe plusieurs facteurs influen\u00e7ant la composition du microbiote\u00a0; les <strong>conditions d\u2019accouchement<\/strong> (par voie basse ou non, pr\u00e9matur\u00e9 ou non) et <strong>la nutrition du nouveau-n\u00e9<\/strong>. En effet, les b\u00e9b\u00e9s allait\u00e9s, qui sont n\u00e9s \u00e0 terme et par voie basse, poss\u00e8dent un <strong>microbiote intestinal g\u00e9n\u00e9ralement plus \u00e9quilibr\u00e9<\/strong>. En effet, la grande majorit\u00e9 des HMO atteint l\u2019intestin intacte. Les mol\u00e9cules y agissent comme <strong>substrats m\u00e9taboliques pour des bact\u00e9ries b\u00e9n\u00e9fiques<\/strong>, les bifidobact\u00e9ries.<\/p>\n<p>En plus de leur r\u00f4le de pr\u00e9biotique, les HMO agissent comme <strong>antimicrobiens.<\/strong> Ils jouent notamment un r\u00f4le <strong>anti-adh\u00e9sif<\/strong>. En effet, ils se fixent sur les cellules \u00e9pith\u00e9liales, ce qui emp\u00eache les pathog\u00e8nes de s\u2019y accrocher qui se retrouvent alors excr\u00e9t\u00e9s sans causer de sympt\u00f4me. Ils peuvent \u00e9galement <strong>modifier les sites de fixation<\/strong> et <strong>bloquer les interactions mol\u00e9culaires<\/strong> permettant aux pathog\u00e8nes de s\u2019accrocher aux cellules \u00e9pith\u00e9liales [4].<\/p>\n<p>Les HMO agissent \u00e9galement en tant qu\u2019<strong>immuno-modulateurs. <\/strong>En effet, ils pr\u00e9sentent une structure d\u2019oligosaccharide similaire \u00e0 celle des ligands des immuno\u2014r\u00e9cepteurs, leur permettant de se lier aux cellules immunitaires. Il en r\u00e9sulte une modification de la production de mol\u00e9cules directement impliqu\u00e9es dans le fonctionnement du syst\u00e8me immunitaire<sup>4<\/sup>.<\/p>\n<p>Enfin, ces oligosaccharides aident au d\u00e9veloppement c\u00e9r\u00e9bral. En effet, les HMO sialyl\u00e9s repr\u00e9sentent une source d\u2019acide sialique, un nutriment n\u00e9cessaire au <strong>bon d\u00e9veloppement du cerveau et des capacit\u00e9s cognitives du nouveau-n\u00e9 [2]<\/strong>.<\/p>\n<p>&nbsp;<\/p>\n<h3>Les HMO dans la formulation des laits infantiles<\/h3>\n<p>Lorsque l\u2019allaitement est impossible, <strong>les formules de lait infantile sont l\u2019unique alternative<\/strong> utilis\u00e9e afin de nourrir le nourrisson. Il est donc primordial que ces produits assurent la <strong>s\u00e9curit\u00e9 alimentaire<\/strong> de l\u2019enfant et garantissent des formules de <strong>bonne qualit\u00e9<\/strong>, couvrant ses <strong>besoins nutritionnels. <\/strong><\/p>\n<p>La pr\u00e9sence des HMO dans l\u2019alimentation infantile permet de r\u00e9pondre aux enjeux cit\u00e9s pr\u00e9c\u00e9demment. Il a notamment \u00e9t\u00e9 d\u00e9montr\u00e9 que l\u2019ajout de 2\u2019FL et de LNnT dans les formulations de lait infantile permettrait un <strong>meilleur d\u00e9veloppement de la microflore des nouveaux n\u00e9s [4]<\/strong>. Cependant<strong>, l\u2019OMS recommande l\u2019allaitement comme unique source d\u2019alimentation jusqu\u2019\u00e0 l\u2019\u00e2ge de 6 mois<\/strong>. Un des enjeux des producteurs de formules de lait infantile est donc d\u2019enrichir celles-ci en HMO, mol\u00e9cules bioactives n\u00e9cessaires \u00e0 la bonne croissance du nourrisson.
